Lion Air crash: Victim's fiancée takes wedding photos alone

Intan Syari's fiance, medical doctor Rio Nanda Pratama, was among 189 people on the Boeing 737 MAX 8 plane who died when it crashed a few minutes after taking off from Jakarta on October 29. Now, investigators are focusing their efforts on figuring out if the plane crashed because of an update to a safety system that was created to pull the plane out of a risky stall, The New York Times reported.

Dennis Tajer, communications committee chairman at Allied Pilots Association (APA), a labour union representing American Airlines pilots, said Boeing exhibited a "failure of the safety culture" by not updating pilots early enough on how the new systems work. Investigators believe the system may have been triggered on inaccurate data transmitted or processed from sensors on the fuselage, causing the plane to nosedive into the water.

Boeing and the Federal Aviation Administration have asked airlines, including United, to better prepare crews to understand the aircraft's automated system.

A safety feature added to the updated 737 Max, which was created to prevent pilots from losing control, has been implicated by Indonesian investigators in a Lion Air jet's sudden dive into the Java Sea after taking off from Jakarta on October 29. In the meantime airlines, pilots, regulators and jet manufacturers have been frantically reviewing flight protocols and systems to ensure passengers on other 737 MAX8 jets are not put at risk.

Even though this problem was - according to investigators - not covered in the operating manual, pilots did have access to a checklist created to turn off errant systems when the plane started nosing downwards at the wrong time, said Soejono, a Lion Air instructor who like many Indonesians goes by one name.

The focus of the investigation appears to be expanding to the clarity of US -approved procedures to help pilots prevent the 737 MAX from over-reacting to such a data loss, and methods for training them. "Take good photos and send them to me".
